1. Product brand (Example: Kirkland, Kraft, Paskez, etc)
2. Product name (Example: 2% Milk, Vanilla Yogurt, Rice Cereal, etc)
3. Product photo (front and back panel required)
4. OPTIONAL: Is the product Kosher certified for the rest of the year? If so, by which agency?
